In each round, the primary goal is to eliminate bandits without harming any hostages. Bandits appear from various locations within the saloon, such as behind signs, from windows, and through swinging doors. Hostages are identifiable as they shake their hands and head to indicate they should not be shot. Accidental hits on hostages result in losing one of three lives. The game emphasizes precision and quick reflexes, requiring players to manually reload their weapon after each shot. The difficulty increases over time as enemies spawn more frequently and in trickier positions, often near hostages.

The game operates as an endless arcade-style survival shooter, where players aim for a high score. Power-ups appear during gameplay, offering temporary advantages or score multipliers. Players can also collect extra lives and bullets to extend the time between reloads, as well as silver and gold bars to boost their score. The game is primarily a single-player experience.
