The game allows players to take control of one of the 24 national teams that participated in the World Cup. Before matches, players can choose formations, adjust lineups, and review player statistics. Matches are played from a top-down perspective, with an emphasis on passing, tackling, and tactical play.

The "Winners Edition" was a special updated version released after the tournament ended. It included the actual World Cup results, final standings, and updated team information, giving players the chance to replay the competition as it really happened.

Although the gameplay was simpler than some of the leading football simulations of its era, the game was praised for its authenticity, large selection of teams, and faithful recreation of the 1990 World Cup.
