Red Ball 5 is a platformer and adventure game where players control a red ball character. The game transports the player to a scary forest filled with witchcraft and monsters. The narrative involves an eternal confrontation between Evil Cubes and Kind Balls, with the hero ball tasked to defeat ordinary enemies and "Cubo Bosses."

The main goal of each round is to find all the stars available on the level, with their number gradually increasing as the game progresses. Players must also keep an eye on the time limit and locate a hidden chest containing a small wreath, which serves as the main bonus. While collecting stars and bonuses, players need to dodge various monstrous inhabitants that can attack the ball. Some enemies, like horned ones, will damage the Red Ball upon contact, while hornless variants can be defeated by jumping on them. The game features 30 levels, each presenting entertaining puzzles to solve and different enemy types.

Red Ball 5 offers a single-player experience where agility is key to navigating the levels and avoiding monsters. The game includes elements of puzzle-solving, often requiring players to move objects like stones and chests to reach higher areas or activate buttons to remove walls. Players are given three lives, indicated by red hearts, and losing them all means restarting from the first level. The game also features a checkpoint system, allowing players to respawn at the last checkpoint passed after dying.
